Botox in Tunisia: what 1 ml treats, when to book, and who should inject it

Botox in Tunisia: the essentials

Botulinum toxin — known everywhere by the brand name Botox — relaxes a muscle so that the lines it creates when you move stop deepening. It is injected in minutes, it wears off, and it does not fill anything.

At ServiHealth it is €300 per millilitre with the consultation included, and 1 ml usually covers three areas. No stay is needed: it is an appointment, not a trip. Injections are given by Dr Marwen Guesmi and Dr Walid Saidane at the Clinique Internationale Myron in Tunis.

Two things on this page are worth more than the price. Timing — the effect arrives between day three and day seven and settles at two weeks, so a session a few days before an event is the one mistake to avoid. And who holds the syringe, which is the difference between a good result and the photographs people send us asking whether it can be undone.

What Botox is, and what it does to a muscle

Botulinum toxin is injected in very small quantities into a specific muscle. It blocks the nerve signal that tells that muscle to contract. The muscle stops pulling, and the line it was folding into the skin softens.

That mechanism explains everything the treatment can and cannot do. It works on lines caused by movement — frowning, raising the eyebrows, smiling. It does nothing at all for volume: it adds no substance, it fills no hollow, and it will not lift skin that has descended. If a line is clearly visible when your face is completely still, botulinum toxin is probably not the answer for that line.

It is also temporary by design. The nerve endings recover, the muscle starts working again, and the effect fades over three to six months. Nothing accumulates and nothing is permanent — which is reassuring if you are nervous, and worth budgeting for if you are not.

How much does Botox cost in Tunisia?

€300 per millilitre, with the consultation and the injection included. No stay, no hospital admission, no extras.

Price here is tied to quantity rather than to the number of lines on your face, which is the honest way to do it. One millilitre normally treats three areas. If you only want one area treated, the quantity is reduced and the price is adjusted at the consultation.

Two treatments need more product and therefore cost more: the masseters, for jaw slimming or teeth grinding, and excessive sweating. Both are quoted individually after the consultation, because the dose depends on the muscle and the area.

Describe the areas you are thinking about and send two photographs — one at rest, one making the expression that bothers you — and you will have an opinion and a quote within 24 to 48 hours.

Which areas are treated

With 1 ml the standard combination is three areas, and they are the three that move most:

  • The forehead — the horizontal lines that appear when you raise your eyebrows.
  • The frown lines — the vertical lines between the brows, often the first ones people notice on themselves.
  • Crow’s feet — the lines at the outer corner of the eye when you smile.

How the millilitre is divided between those three is decided at the consultation, not in advance, because faces are not symmetrical and nobody uses all three muscle groups equally. If you have a strong frown and a quiet forehead, the product goes where the movement is.

You can also ask for a specific movement to be preserved. Patients who are on camera, or who simply dislike a motionless forehead, often ask for the forehead to be treated lightly so that it still moves. Say so at the consultation — it is a normal request and it changes the dosing.

Botox is not only for wrinkles

Some of the most useful things botulinum toxin does have nothing to do with wrinkles, and they are the reason a fair number of patients come.

Masseters: jaw slimming, and teeth grinding

The masseter is the chewing muscle at the angle of the jaw. Treated, it reduces in volume gradually over several weeks, which softens a square lower face. The same injection usually reduces bruxism — less clenching at night, less jaw pain, fewer morning headaches. Plenty of people come for the headaches and keep the jawline as a bonus.

The timescale is different here: allow 6 to 8 weeks to see the change, and at least two months before any event. It is not a quick fix.

Excessive sweating

Underarms, palms and soles. The effect lasts 6 to 9 months — considerably longer than on wrinkles — which makes it one of the better-value uses of the product and one that genuinely changes daily life for people who have it badly.

Smaller, precise uses

  • A drooping brow tail — a slight lift at the outer eyebrow. The effect is modest, and anyone promising you a brow lift from a syringe is overselling it.
  • Vertical neck bands (the platysma muscle).
  • A gummy smile.

Masseters and sweating both need more product than a standard facial treatment, so they are priced separately after the consultation.

Botox or filler: do not confuse them

These two are constantly confused, and choosing the wrong one is the most common reason people are disappointed by injectables.

  • Botox relaxes. It stops a muscle pulling, so lines caused by movement soften. It adds nothing.
  • Filler fills. Hyaluronic acid restores volume where something has hollowed or a line has become permanent.

The practical test is simple: look in the mirror with your face completely still. A line that is still clearly there at rest, or a hollow, is a filler question — see dermal fillers. A line that appears or deepens when you move is a Botox question.

They are often complementary, and a face is frequently best served by a little of each. What neither will do is lift skin that has descended. If what bothers you is sagging rather than lines, the honest answer is thread lifting or surgery, and injecting more product will only make that clearer.

Are you a candidate for Botox?

Who it suits

People whose lines appear or deepen with movement — frowning, raising the brows, smiling. That is the whole indication, and it is a wide one.

Contraindications

  • Pregnancy or breastfeeding.
  • A neuromuscular disease, myasthenia gravis in particular.
  • An active skin infection in the area to be treated.
  • A known allergy to any component.
  • Certain medications, notably aminoglycoside antibiotics, which interact with the toxin.

Bring a complete medication list to the consultation, including anything taken occasionally. The interaction above is the reason this is not a formality.

Adults only

Botulinum toxin for cosmetic purposes is given to adults. In the UK it has been illegal since 2021 to administer botulinum toxin or dermal fillers for cosmetic reasons to anyone under 18, and that reflects a position shared well beyond the UK. It is not a treatment for a face that has not finished changing.

On its own, or during a trip you are already making?

Here is the advice a clinic is not supposed to give you: if you live in Europe or Africa, do not fly to Tunisia purely for an injection. The journey will cost more than the treatment, and you can have it done competently much closer to home.

There are two situations where it genuinely makes sense.

During a trip you are already making

If you are coming for surgery, adding a session is easy — but the sequencing matters. It goes before the operation, or several weeks after. Never in the days following facial surgery, when swelling makes it impossible to judge muscle movement and the injection cannot be placed accurately.

Alongside another injectable treatment

Combined with filler or thread lifting on the same day, which is a common and sensible pairing.

If you live in Tunisia or you are here regularly, none of this applies — it is simply an appointment, with a top-up every three to six months.

When to book before an event

This is where most of the regret comes from, so the numbers are worth reading twice.

  • Three weeks before, at the absolute minimum.
  • Ideal window: 3 to 6 weeks before.
  • If it is your first time, allow two months, so that there is room for an adjustment at the two-week review.
  • Masseters: at least two months, because the change takes 6 to 8 weeks to appear.

Why not a few days before

Two reasons, and both are mechanical rather than cautious. The effect has not arrived — nothing happens in the first 48 hours and the result only settles at two weeks, so you would attend your event with the injection done and nothing to show for it. And small bruises are possible for a few days; they cover with make-up, but you would rather not be covering anything.

Tell us the date of the event in your very first message. It is the single piece of information that determines whether we book you in or tell you to wait.

Your appointment, step by step

  1. First contact. Describe the areas that bother you and send two photographs: your face at rest, and your face making the expression concerned. Natural light, no filter. You receive an opinion and a quote within 24 to 48 hours.
  2. Preparation. For the three days before, avoid aspirin, anti-inflammatories, blood-thinning supplements and alcohol, unless your own doctor advises otherwise. All of them increase bruising.
  3. Arrival. You are met at Tunis-Carthage and taken to the Clinique Internationale Myron. Consultation and injection the same day.
  4. Consultation. The doctor examines your face at rest and in movement, decides the areas and the number of units, and this is where you say which movements you want kept — a forehead that still moves, for example.
  5. The injection. A very fine needle, a series of quick stings, 15 to 20 minutes. You sit upright with your eyes open. Numbing cream is available if you want it; allow 20 minutes extra.
  6. Straight afterwards. You leave immediately. For the next 4 hours: do not lie down, do not massage your face, and avoid intense exercise.
  7. The following days. The effect appears between day 3 and day 7 and settles at two weeks. Light bruising is possible and covers with make-up.
  8. Two-week review. Photographs at rest and in expression, and an adjustment if one is needed. This review is the point of a measured first dose.
  9. Around month four, we contact you about a top-up.

Is Botox safe?

Botulinum toxin has been used medically for decades and is well understood. Everything that goes wrong with it in practice comes down to dose, placement and who is holding the syringe.

Normal and expected

Small red dots that disappear within hours. Light bruising for a few days. A sensation of heaviness in the forehead during the first week. Mild, short-lived headaches.

Possible unwanted effects

  • Asymmetry between the two sides.
  • An eyebrow that rises oddly.
  • Ptosis — a slightly drooping upper eyelid.
  • A frozen forehead, from too much product.

All of these are temporary. They disappear as the product does, over a few weeks to a few months. Unpleasant, not permanent — which is genuinely the most reassuring thing about this treatment.

Who injects, and why it is the whole question

Only a doctor, in a medical setting. Beauty salons, home visits and group “parties” are where the accidents come from, here and in every country. The product is a prescription medicine, the anatomy underneath is millimetre-precise, and nothing about a low price compensates for an injection placed in the wrong muscle.

How a first session is handled here

A measured dose, deliberately. Units can be added two weeks later; they cannot be taken out. Any doctor who loads your first session to impress you has removed your only safety margin. That, plus examination in movement and a review at two weeks, is the entire method.

After the session

There is no downtime and no dressing. You walk out and carry on with your day. The only rules are for the first four hours.

For 4 hours

  • Do not lie down.
  • Do not massage or rub your face.
  • No intense exercise.
  • No nap — and if you are flying straight back, do not fall asleep with your head against the window.

The reason behind all four is the same: the product needs to stay where it was placed. Pressure and gravity in the first hours can move it into a neighbouring muscle, which is how an unintended drooping eyelid happens.

Afterwards

Flying is fine with no special precaution. Make-up can go over any small bruise. Nothing changes in the first 48 hours, so do not judge anything before day three — and do not go back for more product because “it has not worked” on day two. It has not worked yet.

Conseils pratiques après une injection de Botox

A temporary effect, to be repeated

Botulinum toxin wears off. On wrinkles the effect lasts 3 to 6 months; on excessive sweating, 6 to 9 months. The muscle recovers and your face returns to exactly what it was.

That last point is worth stating plainly, because it is the most common worry: stopping does not make anything worse. There is no rebound and no accelerated ageing. You simply go back to the face you would have had. The lines may look more noticeable to you, but that is the comparison with the treated version, not damage.

In practice most people settle into a top-up every three to six months. If you live abroad, build that into the decision from the start: it is a recurring appointment, not a one-off. And a top-up given before the previous one has fully worn off generally needs less product rather than more.

The doctors

Injections at ServiHealth are given by Dr Marwen Guesmi and Dr Walid Saidane, doctors practising aesthetic medicine, at the Clinique Internationale Myron in Tunis.

The consultation includes examining your face in movement, not only at rest — which sounds obvious and is routinely skipped. It is the only way to see which muscles are actually producing the lines that bother you, and it is the difference between a treatment plan and a standard three-area template.

Consultations are held in English and French.

Why Tunisia?

  • It is an appointment, not a trip. No stay, no admission, back out the same day.
  • Travel. Direct flights of 1 hour 30 to 3 hours from most of western Europe into Tunis-Carthage, with regular connections from West Africa. EU and Swiss nationals need no visa; every other passport should check its own requirements, which change.
  • A doctor injects, in a clinic. Given how much of this treatment is sold in non-medical settings worldwide, that is not a small distinction.
  • English and French throughout.

And the honest caveat

As set out above, this is not a treatment worth travelling for on its own. It makes sense during a trip you were making anyway, alongside another treatment, or if you are here regularly. Said plainly because it is true, and because a clinic that tells you to fly three hours for a fifteen-minute injection is not thinking about you.

Before you come: a checklist

Documents

  • A valid passport, and a visa if your nationality requires one.

Photographs to send in advance

  • Face at rest, and face in expression — brows frowned, then smiling. Natural light, no filter.

Health

  • A full list of medication and allergies, flagging in particular antibiotics, anticoagulants and any neuromuscular condition.
  • Previous Botox or filler: dates, areas and how you felt about the result. This genuinely changes the dosing.

Three days before

  • Avoid aspirin, anti-inflammatories, vitamin E, ginkgo and alcohol — all of them increase bruising.

On the day

  • Come without make-up. Allow an extra 20 minutes if you want numbing cream.

Your calendar

  • Mention any event in your first message. At least three weeks before, ideally three to six, and two months for a first session or for masseters.
  • For the 4 hours after: no sport, no massage, no lying down and no nap. Stay upright.
  • Put a reminder in your diary for around month four.

Frequently asked questions about Botox Injections in Tunisia

Nothing bad. The muscle recovers, the movement returns and your face goes back to exactly what it would have been. There is no rebound, no acceleration of ageing and nothing accumulates. The lines may look more noticeable to you, but that is the comparison with the treated version rather than damage. This is the most common worry about the treatment and it is unfounded.

Yes. One millilitre normally covers three areas — forehead, frown lines and crow’s feet — but if you only want one treated, the quantity is reduced and the price is adjusted at the consultation. Pricing here follows the amount of product used, not the number of lines on your face.

Botox relaxes a muscle so that lines caused by movement soften. Filler adds volume where something has hollowed or a line has become permanent. The test: look in the mirror with your face completely still. A line still clearly there at rest, or a hollow, is a filler question. A line that appears or deepens when you move is a Botox question. They are often complementary, and neither lifts skin that has descended.

15 to 20 minutes, with no anaesthetic. Numbing cream is available if you want it, applied about 20 minutes beforehand, so allow the extra time. You sit upright with your eyes open and you leave immediately afterwards — there is no downtime and no dressing.

Usually three areas: the forehead, the frown lines between the eyebrows, and the crow’s feet at the outer corners of the eyes. How the millilitre is divided between them is decided at the consultation, because nobody uses those three muscle groups equally. Masseters and excessive sweating need more product and are quoted separately.

Barely. The needle is very fine and what you feel is a series of quick stings over 15 to 20 minutes. No anaesthetic is used as standard; numbing cream is available on request and takes about 20 minutes to work. Small red dots appear at the injection points and disappear within hours.

€300 per millilitre, consultation and injection included, with no stay required. One millilitre generally covers three areas; treating a single area reduces the quantity and the price. Masseter treatment and injections for excessive sweating need more product and are quoted individually after the consultation.

No. It is an appointment, not a trip: consultation and injection on the same day, and you leave straight afterwards. A same-day return is sometimes possible from Europe. For the four hours after the injection, stay upright — which means not falling asleep against the aeroplane window on the way home.

Yes, by treating the masseters — the chewing muscles at the angle of the jaw. They reduce in volume gradually, softening a square lower face. The change takes 6 to 8 weeks to appear, so allow at least two months before any event. The same injection usually reduces teeth grinding, jaw pain and morning headaches.

They slim different things. Masseter Botox reduces a muscle at the angle of the jaw — reversible, repeatable, and it wears off. Buccal fat removal takes out a fat pad in the cheek and is permanent. If your lower face is wide because the chewing muscle is strong, Botox is the answer; if it is the cheeks that are full, it is not. The examination settles which, and it is worth settling before choosing the irreversible option.

Yes, and it is one of its better uses. Injected into the underarms, palms or soles, it reduces sweating for 6 to 9 months — considerably longer than its effect on wrinkles. It needs more product than a facial treatment, so it is quoted separately after the consultation. For people who have hyperhidrosis badly, it changes daily life rather than appearance.

Between day 3 and day 7. Nothing at all changes in the first 48 hours, and the result is fully settled at two weeks. Do not judge it before day three, and do not go back for more product on day two because it "has not worked" — it has not worked yet. A review at two weeks is when an adjustment is made if one is needed.

Three weeks at the absolute minimum, with 3 to 6 weeks the ideal window. For a first session allow two months, so there is room for an adjustment at the two-week review. For masseters allow at least two months, because the change takes 6 to 8 weeks. Never a few days before: the effect will not have arrived and small bruises are possible. Mention the date in your first message.

Light bruising is possible for a few days and covers easily with make-up. You reduce the risk considerably by avoiding aspirin, anti-inflammatories, vitamin E, ginkgo and alcohol for the three days before, unless your own doctor advises otherwise. Small red dots at the injection points are different — they disappear within hours.

For four hours: do not lie down, do not massage or rub your face, avoid intense exercise and do not nap — including against the window on the flight home. The reason for all four is the same: the product must stay where it was placed, and pressure or gravity in the first hours can move it into a neighbouring muscle, which is how an unintended drooping eyelid happens. After that, normal life, and flying is fine.

It can, from too much product, and it is temporary like everything else — it wears off over a few weeks to a few months. It is avoided by a measured first dose: units can be added two weeks later, they cannot be taken out. You can also ask explicitly for specific movement to be preserved, a forehead that still moves for instance. Say so at the consultation; it is a routine request and it changes the dosing.

Expected and harmless: small red dots that fade within hours, light bruising for a few days, a heavy feeling in the forehead during the first week, and mild short-lived headaches. Possible unwanted effects: asymmetry, an eyebrow that rises oddly, ptosis (a slightly drooping upper eyelid), or a frozen forehead from overdosing. All of them are temporary and resolve as the product does, over a few weeks to a few months.

A doctor, in a medical setting — and that is not a formality. Beauty salons, home visits and group sessions are where the accidents happen, in every country. Botulinum toxin is a prescription medicine, the anatomy beneath the skin is millimetre-precise, and no price difference compensates for a product placed in the wrong muscle. In the UK it has also been illegal since 2021 to give it for cosmetic reasons to anyone under 18.

Honestly, no. If you live in Europe or Africa the journey costs more than the treatment and you can have it done competently closer to home. It makes sense in two cases: during a trip you are already making for something else — placed before the surgery or several weeks after, never in the days following facial surgery — or alongside filler or thread lifting on the same day. If you live in Tunisia or come here regularly, it is simply an appointment every three to six months.
