Penis Enlargement Surgery in Tunisia: Girth, Length and Realistic Results

Penis enlargement surgery in Tunisia: the essentials
Two different operations are offered under this heading, and they have almost nothing in common.
- Girth augmentation uses your own fat. A small liposuction harvests it, it is purified, and it is injected under the skin of the shaft. €1,850.
- Lengthening releases the suspensory ligament, which holds part of the shaft inside the body, so that more of what already exists sits outside it. €2,000.
- Both together, in one operation. €2,850.
All three carry a 3-day stay, take one to two hours under deep local or light general anaesthesia, and are performed at the Clinique Internationale Myron in Tunis.
The honest part, which belongs at the top rather than the bottom. Lengthening is most visible at rest; in erection the gain is limited, and the angle of erection sits lower afterwards because the ligament that held it up has been released. Girth augmentation works, but part of the transferred fat is reabsorbed and the proportion cannot be predicted. Neither operation delivers a figure anyone can quote you in advance, and this page quotes none.
One thing matters more than everything else here and has a section of its own below: liquid silicone and paraffin must never be injected. They are offered to men in this situation, sometimes by people in clinical settings, and they cause damage that is frequently permanent. ServiHealth does not use them under any circumstances.
And because it is usually the first question rather than the last: nobody here will ask you to justify why you are consulting. How confidentiality works in practice — your invoice, the airport, your photographs — is set out further down.
A warning about dangerous injections
This is the most important section on the page, and it has nothing to do with choosing a clinic in Tunisia rather than anywhere else.
Liquid silicone and paraffin must never be injected into the penis. Not by a surgeon, not in a clinic, not anywhere, at any price.
What they do is not a rare complication, it is what these substances do:
- They migrate through the tissue away from where they were placed, and cannot be reliably retrieved.
- They provoke chronic inflammation and hard masses that distort shape permanently.
- They cause skin breakdown, infection and deformity, sometimes years after the injection.
- Removing them means extensive surgery that frequently cannot restore normal appearance or function, and sometimes means removing skin with the product.
They are offered because they are cheap, quick and require no operating theatre. Men accept them because the alternative seems expensive and intimidating. The arithmetic is wrong in both directions: the injection costs a fraction of surgery, and correcting what it does costs far more than surgery ever would, if it can be corrected at all.
If anyone offers you a permanent filler injection for this, refuse and leave the building. That applies to a clinic, a salon, a hotel room and a recommendation from someone who had it done. It applies regardless of what the substance is called.
At ServiHealth, girth augmentation is performed with your own fat and nothing else. It has its own limitation, which is that part of it is reabsorbed. That is a disappointment you can live with. The other is not.
Girth or length: what is the difference?
They are two unrelated operations addressing two different things. One adds tissue; the other repositions tissue that is already there.
| Girth augmentation | Lengthening | |
| What is done | Your own fat is harvested, purified and injected under the skin of the shaft | The suspensory ligament is released, exposing shaft that was held inside the body |
| Material | Your own tissue — nothing foreign | Nothing added |
| Main limitation | Part of the fat is reabsorbed, in a proportion nobody can predict | The gain shows mainly at rest, and the erection angle sits lower |
| Donor site | Yes — a small liposuction, usually the lower abdomen | None |
| Price | €1,850 | €2,000 |
Most men arrive with one of the two in mind and discover at the consultation that the other is closer to what they actually described. Say what bothers you rather than naming a procedure, and let the examination decide which operation answers it — or whether either does.
No figure for gain appears on this page, in centimetres or otherwise, because no honest one exists in advance. Anyone quoting you one is quoting an average at best and an invention at worst.

An honest note about lengthening
Lengthening is the procedure most often misunderstood, and the misunderstanding is predictable enough to be worth setting out before you book rather than after.
The operation does not create new tissue. A portion of the shaft is held inside the body by the suspensory ligament. Releasing that ligament allows more of it to sit outside. Nothing is added; what exists is repositioned.
Three consequences follow, and all three are expected rather than complications:
- The change is most visible at rest. That is where the repositioning shows.
- In erection the gain is limited. Men who imagine the flaccid change translating directly into an erect one are the ones who end up disappointed.
- The angle of erection sits lower. The ligament that held it up has been released, so it no longer does. This is not a side effect that sometimes happens; it is what the operation does.
There is also a longer-term point: scar tissue can retract over time and reduce part of the gain. How much varies, and it is one more reason no figure is quoted anywhere on this page.
None of this means the operation is not worth having. For a man whose concern is principally about appearance at rest, it does what it says. It means the question to settle before you travel is which of the two states actually bothers you — and if the answer is the erect state, lengthening is probably not the operation you are looking for.
A surgeon who tells you that is doing you a service. One who agrees with whatever you hoped for is not.
Can both be done in one operation?
Yes. Girth augmentation and lengthening combined cost €2,850, with the same 3-day stay — against €3,850 if the two were priced separately. One anaesthetic, one theatre session, one recovery.
That said, wanting both is not the same as being a candidate for both, and the combination is decided at the consultation after an examination rather than chosen from a price list.
Two things worth thinking about before you ask for it:
- Recovery is a single recovery, but it is the longer of the two. You are healing a ligament release and a fat graft at the same time, plus the donor site where the fat was taken.
- The two results arrive on different timetables. The lengthening is apparent early; the girth result is not settled until three to six months, once resorption has finished. Judging the whole thing at week six will give you a misleading picture of both.
If your surgeon recommends doing one and reassessing later, that is a clinical judgement rather than a missed sale.
Confidentiality: what it means in practice
"Total discretion" means nothing on its own. Here is what it actually consists of, so you can check it rather than trust it.
| What | How it works |
| Your reasons | Nobody asks you to justify why you are consulting. Not at first contact, not at the clinic. |
| How we contact you | You choose the channel, and you can ask that no sensitive wording appears in any message we send. |
| Your photographs | Sent to the surgeon only. Never published. Deleted on request. |
| At the airport | You are met without a sign bearing your name, by an arrangement agreed in advance. |
| Your invoice | Issued under a general heading rather than naming the procedure. |
| Your documents | Handed to you in person rather than posted. |
| Clinic staff | Bound by medical confidentiality. |
| Anyone travelling with you | Told only what you authorise, and nothing else. |
If anything needs adjusting for your situation — a different contact method, a particular wording, something that must not appear on a document — say so at the first exchange. It is far easier to set up in advance than to correct afterwards.
Why have the procedure in Tunisia?
Tunisia has run an organised medical travel sector for decades: private clinics built around international patients, surgeons trained to European standards, and costs that reflect Tunisian overheads rather than shortcuts in the operation.
What matters specifically for this procedure:
- Privacy is easier away from home. No local waiting room, no appointment in a diary someone else sees, an invoice under a general heading. For most men who enquire, this is a large part of the reason.
- It fits a short trip. Three days covers the consultation, the operation and the first check.
- Distance. Two to three hours from most of Western Europe, with direct connections from several Gulf hubs.
- English throughout, from the first exchange to the follow-up after you are home.
- A proper operating theatre. This matters more than it sounds. The alternative many men are offered — an injection somewhere informal — is the single worst decision available in this field.
Two things to weigh honestly before deciding:
- The girth result is not settled for three to six months, and you will spend that time at home. Know who you contact, and that they will still answer in month four.
- Your examination happens on arrival. The remote opinion is based on what you describe. If the surgeon concludes that one of the two operations is not appropriate for you, or that neither is, you will have travelled for an answer. Ask us in advance what happens financially in that case.
And the criterion that should not decide this: price. The gap between a good result and a bad one here is judgement, and the worst outcomes in this field are bought cheaply.
Your surgeon
This procedure is performed by Dr Taher Djemal, plastic surgeon, practising in Tunisia since the early 1990s, at the Clinique Internationale Myron in Tunis.
You meet him in person for the consultation before anything is decided, and the plan is confirmed, adjusted or reconsidered at that point.
Questions worth asking, and they are short:
- Based on the examination, which of the two operations actually addresses what I described?
- What should I expect to see at rest, and what should I expect to see erect?
- How much of the transferred fat do you expect to remain, and what happens if it is less?
- What will the angle of erection be afterwards?
- What would make you advise against operating on me?
The second question is the one that separates a good consultation from a sales conversation. A surgeon who answers it in two parts — one for each state — is being straight with you. One who answers it in general terms is not.
What to do, which operation, and whether to operate at all is a medical decision and belongs to the surgeon, not to ServiHealth.
Your pathway, and what to prepare
ServiHealth organises and coordinates the stay. The medical decisions belong to the surgeon and the anaesthetist; everything around them is our work.
| Stage | What happens |
| First contact | You describe what concerns you and choose how we contact you. No one asks you to justify it. A first opinion follows within 24 to 48 hours. |
| Quote | In writing, under a general heading, setting out which operation is proposed and what the price covers. |
| Preparation | Pre-operative tests in your own country, following the protocol you are given. Stop smoking for the period your surgeon specifies. |
| Arrival | You are met at Tunis-Carthage without a name sign, by arrangement agreed in advance. |
| Consultation | Examination by the surgeon. Which operation, and what to expect at rest and in erection. The anaesthetic assessment follows. |
| Procedure | One to two hours under deep local or light general anaesthesia. |
| Before you fly | A post-operative check, your care instructions and documents handed to you in person. Three days in total. |
| After you return | Remote follow-up with a contact you can reach. The result is assessed at three to six months. |
What to prepare
- A valid passport, and a visa if your nationality requires one for Tunisia.
- Pre-operative tests, done at home, sent in advance.
- A full list of medication and allergies, anticoagulants included, plus any clotting disorder.
- Stop smoking for the period your surgeon specifies. It affects how a fat graft survives and how a wound heals.
- Loose clothing and cotton underwear for the journey home.
- About a week without physical effort at work after you return, and three to four weeks before sport or sexual activity.
- Your questions, written down. They are harder to ask in the room than at home.
How much does penoplasty cost in Tunisia?
Girth augmentation €1,850, lengthening €2,000, both together €2,850 — each with a 3-day stay.
What the price covers: the consultation with the surgeon, the procedure, the anaesthesia, the transfers, and the post-operative follow-up after you return home.
Quoted separately: a second fat transfer session, if one is discussed from around six months. It is not systematic, and it is not included.
Ask for the inclusions in writing for your own case before you commit, and read them against what you will actually need: flights, the nights you are staying, and the pre-operative tests you have done at home.
Two notes on comparing prices for this particular procedure.
Doing both together saves €1,000 over having them priced separately, which is a genuine saving and not a reason in itself to have both. Which operations are appropriate is a clinical question, not a budgeting one.
And price should be well down your list of criteria here. The cheapest options available in this field are not cheaper versions of this operation — they are permanent filler injections performed outside proper medical settings, and what they cost is set out in the warning section above. There is no version of this where saving money is the right objective.
| Procedure | Price | Stay |
|---|---|---|
| Girth augmentation (fat transfer) | 1850 € | 3 days |
| Lengthening (suspensory ligament release) | 2000 € | 3 days |
| Girth augmentation and lengthening together | 2850 € | 3 days |
Who is not a candidate?
The procedure is for adults of 18 and over, in good health, after an examination. Development must be complete, and no reputable surgeon operates on a minor for this.
The operation is postponed or ruled out in these situations, each assessed individually by the surgeon and the anaesthetist:
- An active local or urinary infection
- Active smoking that has not been stopped beforehand — it directly affects how a fat graft survives
- A clotting disorder, or treatment with anticoagulants
- Any condition that contraindicates anaesthesia
- An untreated urological condition, which is assessed first
- Insufficient fat to harvest, for girth augmentation
And one that is not medical
Unrealistic expectations are a reason not to operate, and the commonest one concerns lengthening: expecting the change seen at rest to appear equally in erection. It does not, and no surgical technique makes it.
There is a second version of this worth naming plainly, once. Many men who consider this surgery have anatomy well within the normal range, and the distress they feel is real but is not about the measurement. If this occupies a great deal of your thinking, if it has persisted for years, if it affects whether you form relationships or undress in front of anyone — that distress deserves to be taken seriously, and surgery is not reliably the thing that resolves it. Talking to a professional about it first is not a brush-off and it is not a diagnosis; it is the step most likely to help, and it does not close the door on surgery afterwards.
That is said once on this page and not repeated. Most men enquiring are not in that position. The ones who are deserve to be told rather than simply operated on, and a clinic that never raises it is not being kind.
What are the risks?
Both operations carry real risks. They belong here, before you book, not in a consent form on the morning.
Expected after-effects, in both: swelling, bruising and tenderness — including at the donor site, if fat has been harvested. These are not complications.
Common to both operations:
- Haematoma
- Infection
- Delayed healing
- Pain that can persist for some weeks rather than days
Specific to girth augmentation:
- Partial resorption of the fat, leaving a result below what was planned. This is the commonest disappointment and the reason a second session exists as an option.
- Surface irregularities and asymmetry
- Firm nodules, uncommonly, where fat has not established a blood supply
Specific to lengthening:
- Scar retraction over time, which can reduce part of the gain
- A lower angle of erection. This is an expected consequence of releasing the ligament, not a complication — but it is a permanent change and you should decide about it in advance.
- A sense of instability during erection, rarely
On erectile function: fat transfer places tissue under the skin of the shaft and does not involve the erectile structures. Raise any concern about erectile function or sensation with the surgeon at the consultation, including anything that pre-dates the operation — it may change what is advisable.
Contact your surgeon the same day if you have worsening pain, a fever, any discharge, marked swelling on one side, or any difficulty passing urine. None of these waits for the next scheduled contact.
Results vary from one patient to another. No gain is guaranteed, in either dimension, and your surgeon will tell you at the consultation what is realistic in your case.
Safety: what we commit to
Commitments are worth something only when they are specific enough to be checked. These are ours.
- Your own fat and nothing else for girth augmentation. No liquid silicone, no paraffin, no permanent filler — under any circumstances, whatever is requested.
- A proper operating theatre, at the Clinique Internationale Myron, with an anaesthetist.
- An examination before any decision. The remote opinion is based on what you describe; the plan is set in person.
- No figure promised in advance. You will not be quoted a gain in centimetres by anyone here, because nobody can honestly quote one.
- A written quote, under a general heading, before you travel.
- A surgeon who may tell you no — to one of the two operations, to both, or to operating at all.
What we do not commit to is a result, and you should be sceptical of anyone who does. Part of the transferred fat is reabsorbed in a proportion nobody can predict. The lengthening shows mainly at rest. Scar tissue behaves differently in different men. These are the honest boundaries of what this surgery does, and a clinic that will not state them is managing your expectations in the wrong direction.
If something concerns you after you are home — a lump, an asymmetry, a result below what was planned — raise it rather than waiting. Most of it is better addressed early, and some of it resolves on its own before the six-month mark.
Recovery and results
Recovery here is less about intensity than about patience: the discomfort is moderate, and it lasts longer than most men expect.
| When | What to expect |
| First days | Swelling, bruising and tenderness, including at the donor site if fat was harvested. Prescribed painkillers. Rest. |
| Around 1 week | Back to work, provided it involves no physical effort. |
| 3 to 4 weeks | Sexual activity and intense sport resume, with your surgeon's agreement. Earlier risks displacing fat that has not yet settled. |
| Several weeks | Residual discomfort can persist. This is normal and not a sign that something is wrong. |
| 3 to 6 months | Swelling fully resolved and fat resorption complete. This is the result. |
| From 6 months | The earliest point at which a second fat transfer session is discussed, if the result is below what was planned. |
Do not judge this before three months. Swelling in the early weeks makes the result look larger than it will be, which means the first impression is misleadingly good and the second, a few weeks later, misleadingly disappointing. Neither is the outcome.
The two results arrive on different timetables if you have had both operations: the lengthening is apparent early, the girth is not settled until three to six months.
On durability: the fat that survives the first months behaves like your own tissue, because it is. Significant weight loss will reduce it, as it reduces fat everywhere. The lengthening holds, subject to the scar retraction described earlier.
If you are considering another procedure during the same stay, read our pages on liposuction or gynaecomastia surgery in Tunisia first. What can reasonably be combined in one session is decided by your surgeon.

Frequently asked questions about Penis Enlargement Surgery in Tunisia
They are two unrelated operations. Girth augmentation uses your own fat: it is harvested by a small liposuction, purified, and injected under the skin of the shaft. Lengthening releases the suspensory ligament, which holds part of the shaft inside the body, so that more of what already exists sits outside it. One adds tissue, the other repositions what is there. They can be done together in a single operation.
Much less than when flaccid, and this is the point on which expectations most often go wrong. Releasing the ligament exposes a portion of shaft that was previously held internally, and that is most apparent at rest. When erect, the gain is limited, and the angle of erection typically sits lower than before because the ligament that held it up has been released. That change in angle is an expected consequence of the operation, not a complication.
Girth augmentation €1,850, lengthening €2,000, and both together €2,850 — each with a 3-day stay. The price covers the consultation, the procedure, the anaesthesia, the transfers and the post-operative follow-up. A second fat transfer session, if one is discussed later, is quoted separately. Ask for the inclusions in writing for your own case before you commit.
Girth augmentation using your own fat is the better-established of the two offered here, because the material is your own tissue and nothing foreign is introduced. Its main drawback is that part of the fat is reabsorbed and the amount is not predictable. Lengthening is a different trade: the gain is real at rest, limited in erection, and comes with the change in erection angle described above. What is never safe, under any circumstances, is liquid silicone or paraffin — see the question on that below.
Yes, and discretion is treated here as a set of arrangements rather than a promise. You choose how we contact you and what may appear in writing. Your photographs go to the surgeon alone and are deleted on request. You are met at the airport without a sign bearing your name. Your invoice carries a general heading. Nobody at any point asks you to explain why you are consulting.
Three days for any of the three options. That covers the consultation, the procedure and the first post-operative check before you fly home. Allow about a week after you return without physical effort at work.
Part of it does, and part is reabsorbed over the first months. The proportion varies from one man to another and cannot be predicted in advance — this page quotes no percentage because there is no reliable figure to quote. What remains after three to six months is what you have. Your surgeon allows for the resorption when deciding how much to place.
Not necessarily. It is discussed from around six months, once the result has fully stabilised, and only if what remains is below what was planned. It is not systematic, it requires another stay, and it is quoted separately. If you are budgeting for an outcome rather than for an operation, it is sensible to know that in advance.
Common to both procedures: haematoma, infection, delayed healing, and pain that can last some weeks. Specific to girth augmentation: partial resorption of the fat leaving a result below expectation, surface irregularities, asymmetry, and occasionally firm nodules. Specific to lengthening: scar retraction that can reduce the gain over time, a lower angle of erection, and rarely a sense of instability during erection. The full list is set out in the risks section of this page, and your surgeon goes through it with you before you consent.
Expect swelling, bruising and tenderness — including at the site where fat was taken, if you are having girth augmentation. Discomfort is managed with prescribed painkillers, and it can persist for some weeks rather than days. Most men describe it as manageable but longer-lasting than they expected.
Fat transfer places tissue under the skin of the shaft and does not involve the erectile structures. Lengthening does change the angle of erection, which sits lower afterwards because the ligament that held it up has been released — an expected consequence rather than a complication — and rarely a sense of instability during erection is reported. Raise any concern about erectile function or sensation directly with the surgeon at the consultation, including anything that pre-dates the operation.
Three to four weeks, with your surgeon's agreement. Strenuous activity and intense sport wait for the same period. Resuming earlier risks the fat being displaced before it has settled, and risks the healing tissue.
Three to six months, once the swelling has fully resolved and the resorption of the transferred fat has finished. Nothing you see in the first weeks is the outcome, in either direction — swelling makes the early result look larger than it will be.
Yes, and that is the €2,850 option, with the same 3-day stay. One anaesthetic and one recovery rather than two. Whether both are appropriate in your case is decided at the consultation after an examination — wanting both is not the same as being a candidate for both.
Because they cause damage that is often permanent and sometimes impossible to repair. Injected into the penis, these substances migrate through the tissue, provoke chronic inflammation and hard masses, cause skin breakdown, infection and deformity, and can require extensive surgery to remove — surgery that frequently cannot restore normal appearance or function. They are not a cheaper version of a legitimate procedure; they are a different thing entirely, offered outside medical settings and sometimes in them. ServiHealth does not use them under any circumstances, and no reputable surgeon anywhere does. If anyone offers you a permanent filler injection for this, refuse and leave.
Because of where the clinic operates rather than what is left out of the operation. The useful comparison is inclusions against inclusions: here the consultation, the anaesthesia, the transfers and the follow-up are inside the price. Ask any clinic for the same breakdown before comparing totals — and on this procedure in particular, price should be well down your list of criteria.
