What is a BBL (buttock fat transfer)?
The BBL uses no implants and no synthetic filler. It is an autologous fat transfer, meaning it is done with your own fat.
The procedure takes place in three stages. The surgeon first harvests fat by liposuction from areas where there is an excess, such as the belly, hips, back or thighs. This fat is then processed so that only the usable cells are kept. Finally, it is injected into the buttocks, at locations defined according to your body shape.
The BBL therefore works on two areas: the silhouette is slimmed where the fat is harvested, and the buttocks gain volume and shape. Because it is your own fat, the result is generally soft and natural to the touch. However, part of the injected fat is reabsorbed by the body in the first months, and the result varies from one patient to another.
Technique variations
The surgeon chooses the method according to your body shape and your goals. In a classic BBL, fat is harvested using fine cannulas. Depending on the case, the surgeon may suggest VASER liposuction, which is ultrasound-assisted, to work more precisely on certain donor areas. J-Plasma can also be added to help the skin retract after liposuction. These options are not suitable for every patient: their benefit is assessed during the consultation.

Lipo 360 and BBL: the most requested combination
Many patients do not come only for buttock volume. What they are looking for is contrast: a defined waist and rounded buttocks. That is exactly what combining a 360 liposuction with a BBL produces.
Lipo 360 treats the abdomen, waist, flanks and back in a single procedure. The fat removed from these areas is then used for the transfer. Nothing is wasted: what is removed from one place is used in another.

What volume can be transferred?
This is the question almost every patient asks, often in the form of a number of cubic centimeters seen on social media.
The achievable volume cannot be decided in advance. It depends on the fat available in your donor areas, the elasticity of your skin and the capacity of the subcutaneous layer to accept the graft. Injecting beyond this capacity does not give a bigger result: poorly vascularized fat is reabsorbed or forms nodules.
Part of the transferred volume always disappears in the first months. This is normal, and it is why no serious surgeon guarantees a precise final volume.
Instead, tell the surgeon the effect you are looking for: a subtle curve or a more pronounced projection. Is the BBL safe? Risks and precautions
Like any surgery, the BBL carries risks. The most serious is fat embolism: fat can enter the bloodstream if it is injected into the gluteal muscle, an area crossed by large blood vessels. To limit this risk, the fat is injected only under the skin, in the subcutaneous layer, and not into the muscle.
Other complications are possible: hematoma, fluid collection (seroma), infection, asymmetry, contour irregularities, reabsorption of part of the fat, healing problems or anesthesia-related complications.
