BBL recovery: what the days and weeks after actually look like
A BBL combines two operations: a liposuction to harvest the fat, and a transfer to reshape the buttocks. Your recovery depends on both — on how much fat was removed, which areas were treated, and whether anything else was done at the same time.
Here is what the days and weeks after actually look like, question by question.
One thing first. This is general information. Your surgeon's instructions take precedence over everything on this page. Where the two differ, follow your surgeon.
What happens in the first few hours?
You wake up sore rather than in sharp pain — aching, tightness, swelling and bruising, mostly across the areas the fat was taken from rather than the buttocks themselves.
ServiHealth plans a five-day stay including three days in hospital at Myron International Clinic in Tunis. That is deliberate: it is the period in which serious complications most often appear, and it is the reason a same-day discharge is not offered. We explain the safety reasoning in our article on BBL risks and safe practice.
The first days: what to expect
The tissues are swollen and your shape may look uneven. This is not the result and it is not a sign that something has gone wrong.
Gentle walking is encouraged from the first days. Avoid sudden movements, lifting anything, and any real exertion.
Can you sit after a BBL?
Not directly on the buttocks, and not for prolonged periods, during the first weeks.
A BBL cushion is included in the package. It sits under the thighs and shifts your weight off the grafted area, and you use it for the flight home as well as at home.
There is no universal timeline here. How long the restriction lasts depends on how much fat was transferred and how you are healing, and your surgeon sets it.
How should you sleep after a BBL?
After a BBL alone, sleeping on your front is generally recommended, with sleeping on your side permitted if your medical team allows it.
If an abdominoplasty was done at the same time, you will be asked to lie slightly bent, which is hard to reconcile with lying on your front — the instruction becomes a compromise between the two. A combined breast procedure changes it again.
This is a good example of why combined procedures are discussed in detail before they are booked.
How long do you wear the compression garment?
The garment works on the donor areas — abdomen, flanks, back — not on the buttocks. Compression on grafted fat is exactly what you are trying to avoid.
A garment that is too tight, or that bunches, does more harm than good. How many hours a day you wear it, and whether you change size as the swelling goes down, are set by your surgeon.
Lymphatic drainage after a BBL
Sessions are often recommended. How many, and how they are spaced, is decided by your surgeon rather than by the therapist.
No pressure on the buttocks. The practitioner works the abdomen, flanks, back and the lymphatic pathways, and leaves the grafted area alone. Tell them you have had a BBL before every session, not just the first, and stop the session if anything becomes genuinely painful.
More on what drainage does and does not do in our guide to lymphatic drainage.
Dressings and the small incisions
The cannulae leave several small incisions. Under the ServiHealth protocol the dressings are generally changed every other day.
Showering is allowed once the medical team confirms it, and the areas are dried carefully afterwards rather than rubbed. Some sutures dissolve on their own — never remove any of them yourself, whatever they look like.
Why do the buttocks lose volume?
Two things are happening at once.
The swelling is going down, so part of what you saw in the first days was oedema rather than fat. And a proportion of the transferred fat is reabsorbed — this happens in every case, without exception, and it is accounted for in how much is injected.
Losing volume in the weeks after the operation does not mean the procedure failed. It means it is too early to be comparing before-and-after photographs.
When is the result final?
- Week 1: significant swelling, bruising and tightness.
- From week 2: the swelling starts to reduce, but it is still there.
- After 1 month: your shape becomes readable.
- 3 to 6 months: the fat has stabilised. This is when the volume can be judged.
- Up to 12 months: the tissues can still settle further.
Checking daily in the mirror tells you nothing and costs you a great deal of peace of mind. Photographs at monthly intervals are more useful than a daily inspection.
What should you eat after a BBL?
A balanced diet with enough protein, and enough water. Avoid very restrictive diets unless there is a medical reason for one — transferred fat needs a stable body to survive in.
In the longer term, significant weight loss reduces the volume you have gained, and significant weight gain changes it too. A stable weight is what protects the result.
When can you go back to work?
This depends more on what your job involves than on how you feel. The main constraint is sitting: a role that means several hours a day in a chair is harder to return to than one that does not.
Plan a genuine convalescence rather than assuming you will manage.
When can you exercise again?
Gentle walking comes well before anything else. Running and weight training require considerably more time, and both are resumed only with your surgeon's agreement.
Does a Vaser liposuction change the recovery?
The after-effects vary with the technique used, the number of areas treated and the extent of the reshaping. Vaser and conventional liposuction do not leave identical recoveries.
Ask which technique is planned before the operation, not afterwards.
What are the warning signs?
Tell us about: sudden pain, unusual or asymmetrical swelling, marked redness, any discharge, fever, or feeling unwell.
Four signs are emergencies rather than calls: breathlessness, chest pain, a racing heartbeat, or pain and swelling in one calf. Seek emergency medical help immediately, including after you are back home, and tell whoever sees you that you have had a fat transfer and when.
Follow-up once you are home
You stay in contact with the team and send photographs at agreed intervals — regularly through the first month, then at defined stages.
These photographs are not a formality. They are how a problem gets spotted from several thousand kilometres away.
Mistakes to avoid
- Comparing your recovery with other people's on social media. You are not seeing their week three.
- Changing or stopping your prescribed treatment without asking.
- Returning to intense activity too early.
- Prolonged pressure on the buttocks, whatever the reason.
- Any aggressive massage of the grafted area.
Preparing a BBL means preparing the weeks after
BBL is offered at €2,950 for a five-day stay including three days in hospital. The package covers the consultation, the procedure, the anaesthetic, the hospital stay, the compression garment, the BBL cushion, transfers and post-operative follow-up. Other prices are on the prices page.
Your file is reviewed before you travel, and the consultation on arrival confirms what was planned, adjusts it, or stops it.
In short: how long does recovery take?
Swelling and aching in the first days, improving steadily over the following weeks. Judge the volume between three and six months, and expect the tissues to keep settling for up to a year.
